Services:

Medicare Savings Programs
Medicare Savings Programs help people with low income and asset levels pay for health care coverage including some or all of Medicare’s premiums and may also pay Medicare deductibles and coinsurance.

Medicare Part D / Low Income Subsidy (LIS)
This program may help persons receiving Medicare Part D with their premiums, co-pays, and deductibles.

Home & Community-based Services
Home care and financial assistance is provided on a monthly basis to prevent out-of-home placement of seniors and persons with disabilities and provides an alternative to nursing homes.

Long-Term Care
This is a program for persons needing help to pay for services in their home or medical facility for more than 30 days. These persons have to meet a medical assessment.

Supplemental Security Income (SSI) Medicaid
This program provides medical benefits for anyone receiving SSI. No application is needed for this program.

Old Age Pension - State Medical Program
OAP provides medical and financial assistance to individuals who have limited resources and who have reached age 60.

Colorado Works / TANF
The Colorado Works Program serves low-income families with children by providing income assistance and supportive services to enable participants to find and retain employment and to provide for their children.

Family Medical Assistance
This is a medical assistance program for eligible low-income residents, including children up to age 19, families, and pregnant women.

Child Health Plan Plus (CHP+)
This program is a low-cost health insurance program for uninsured Colorado children ages 18 and under whose family earnings do not qualify them for Medicaid but they cannot afford private insurance.

Colorado Child Care Assistance Program (CCCAP)
The Colorado Child Care Assistance Program (CCCAP) provides day care financial assistance to families who qualify.

1. Make an appointment - Always attempt to make an appointment. Oftentimes this will save you hours waiting in lines. When on the phone make sure you ask for the documentation needed in order to receive assistance.

2. Do not be late for your appointment - This tip should speak for itself.

3. Dress appropriately - If you want to be treated with respect, dress as appropriately as possible. We are not saying wear a suit. Just make sure you and your clothes are clean and non offensive. We have found that clients who dress in this manner have a better chance of receiving aid.

4. Bring all pre determined documentation - Bring all of the documents you were advised to bring from your appointment phone call. Without them, you will not receive aid.
