Must be resident of Forsyth or Stokes County.

Must provide:

Picture ID
Social Security Card or I.T.I.N. Number
Proof of crisis
Proof of household income for last and current month

As many bills and receipts from last and current month as possible to show how the household money has been spent

There are no set limits on the number of prescriptions filled or the value of medications given to one client.
There are no set financial guidelines for eligibility, but the client must demonstrate a true financial need.

Tips For Prompt And Professional Assistance
1. Make an appointment - Always attempt to make an appointment. Oftentimes this will save you hours waiting in lines. When on the phone make sure you ask for the documentation needed in order to receive assistance.
